No matter where you are, our care coordinators will use the nH Predict|Function assessment and their years of clinical experience to partner with your care team and design a care plan that addresses your unique health needs to help you recover faster, get home, and stay there. What You Can Expect

Our goal is to help you return home to improve and maintain your health to the best of your ability. That starts with your personalized care plan. Depending on your care plan, a care coordinator might visit with you in the hospital, in a transitional care setting, or in your home, or he or she might coach you through your therapies over the phone. And, you can always call us. No matter where you are, you will have an ongoing, personal relationship with one of our clinically trained care coordinators who is invested in you.

The specific things we do will depend on your specific needs, but you should expect regular visits or phone calls from your care coordinator to review your progress and make any adjustments to your care plan. Your care coordinator can help clarify your discharge instructions, make sure you have all of the medications your doctor prescribed, coordinate your follow-up appointments, and help you obtain resources and support in your community. Together, we will make sure nothing falls through the cracks.


How Do I Find…?

During our time together, our care coordinators help you navigate your post-acute care needs, but you or your loved ones may still have questions or concerns. Your physician should answer questions about your medical care and course of treatment, but what about questions related to non-medical care, supplies, or support networks?

If you do not, ask your health plan or the doctors in your hospital about how you can access naviHealth care coordination services.
